Transitional services assist individual’s transition into the community whether it be from nursing facilities and other institutions. This service assists individuals by transitioning them from long-term care facilities into a community setting of their choice. ILC facilitates this process by acting as an advocate to facilitate this transition, while working with the individual and various agencies and services. ILC has access to Transition Funds to assist individual when needed. This core service seeks to assist individuals by:
· Assisting individuals navigate significant life transitions
· Relocating from a nursing home or institution into a community-based living arrangement
· Assisting individuals at risk of moving into a nursing home or institution to remain in the community
· Assisting young adults as they transition from high school into adulthood
