All individuals, regardless of age or disability, want to choose where to live. Most individuals choose to live in their homes and communities until it is no longer possible or safe to do so. The Independent Living Center assists individuals and families to transition from long-term care facilities, nursing homes and assisted living homes back to their homes and communities to ensure the move is as problem-free as possible. We can assist with the actual transition and continue to provide advocacy and service coordination to ensure that the individual receives the services to which he or she are entitled.
Transitions between grades and schools can be difficult for some students with disabilities, especially as they near graduation. If you or a family member is a high school student nearing your completion you are likely considering what is next. Will you enter the work force or go to college or a trade school? High school students with disabilities have the same opportunities as everyone else. The transition from high school to a next step can be an intimidating one. ILC can offer support to students with disabilities that are nearing completion of high school or GED. We can help you figure out what reasonable accommodations look like in a post high school environment and what services and benefits are available to you to help you plan for success.
